Student Health Information

This information is provided on your child’s Student Registration Form. This is done digitally and part of MyInnovOak. Please review and update the information at the start of each year and ensure it is kept accurate.

If there is a serious condition that we should be aware of immediately, please contact your child’s teacher. By keeping us up to date you will help us attend to your child’s needs, particularly in an emergency situation.

Please check your information periodically during the school year to ensure it is timely.

Areas that should be reviewed:

• Parent contact information during the school day

• Full contact information for all parents (phone/cell, email address etc)

• Alternative work contacts as required

• Emergency Contact information

• Change of address, phone numbers etc

• Change of custody

• Change in health or other medical issues
