Medication - (Prescription and Non-Prescription)

Some students are allergic to various medicines including penicillin and aspirin. Because of our concern for student safety, we cannot take the chance of any drugs/medication falling into the wrong hands. Therefore, no drugs/medication of any kind are allowed at school unless previous arrangements have been made in advance with the office.

If your child must receive medication at school, the form “Consent to Administer Medication” MUST be completed and signed by the parent. This procedure applies to all types of medication. The form is available at the school office.

  • Form 3004a gives is to provide the school with parental consent

  • Form 3004b documents the medication, dosage, sideffects of the medication being administered

If the school is administering a mid day dosage or a medication more than one day, we ask that the medication remain at school and that it is not transported by the student in most cases.

Parents often split the containers so that medication is available at home as well. We ask that you send all medication in its original containers so that there is no confusion around dosage or who the medication is for
